GENERAL RELEASE FORMS

Adult Release Form

If you are 18 or older and are involved in a New Life sponsored activity, you will need to fill out this waiver. Check with the leader of your activity for more information.

Minor Release Forms
All minors under the age of 18 will need a parent or legal guardian to fill out these release forms to be able to participate in New Life sponsored activities. Check with your child's leader for more information.

DEPARTMENT HEAD FORMS:

Multi-Use Form

This all-in-one form allows you to request that your event be approved for the calendar. You will also be able to specify other requests, including childcare, technical services, and media & design development for your event.

Bulletin Request Form
Once your event is approved, you may fill out a request form for promotion of your event in the bulletin. Ads may be edited for space and will generally run for two weeks. Bulletin forms must be submitted 3 weeks prior to the bulletin date you choose.
