Minor Release Forms
All minors under the age of 18 will need a parent or legal guardian to fill out these release forms to be able to participate in New Life sponsored activities. Check with your child's leader for more information.

This all-in-one form allows you to request that your event be approved for the calendar. You will also be able to specify other requests, including childcare, technical services, and media & design development for your event.

Bulletin Request Form
Once your event is approved, you may fill out a request form for promotion of your event in the bulletin. Ads may be edited for space and will generally run for two weeks. Bulletin forms must be submitted 3 weeks prior to the bulletin date you choose.
